What does "the Employer to which this request applies” refer to? Print

This question refers to the Employer under whom the 403(b) was set up. This may be your current Employer or your previous Employer, depending on the type of request. Even if a participant is retired and no long has an Employer, they need to fill in this line with the name of the Employer they were working with when they were making contributions to their 403(b). Only the first line of Section A deals with the Employer. All the other information in Section A is the participant’s information: their name, address, phone number, date of birth, social security number, etc.
